The application form is not to be used for Overseas Students studying on a student visa

How to complete the form

  • Complete all necessary personal and course details.
  • Identify the category (as detailed in the Reassessment of Course Fees Policy), under which you are applying for a reassessment and give details of the reason for your request.
  • State your last date of attendance or date of change in circumstances.
  • Sign and date the statement.
  • Ensure that any necessary supporting documentation is enclosed with your application form.
